Surgery closure for training

The Practice will be closed from 1pm on Tuesday 18th March, and will reopen on Wednesday 19th March at 8am.

Private letter request

Use this service to request a private letter.

You can use this service if you:

  • are registered at the surgery

Timescale

Please note, these requests involve the GP reviewing your medical records, dictating the letter, and the administrative team subsequently typing and printing it. While we strive to fulfil all requests promptly, it is important to understand that private letters and reports do not take precedence over NHS commitments.

Kindly allow up to 28 working days, from the date we receive payment, for us to complete your request.

Content

It is crucial to note that we cannot accept requests for the doctor to write specific words verbatim. The content of the letter will be based on the GP’s knowledge supported by your medical records. Both the letter and the accompanying opinion are considered final, and requests for alterations cannot be accommodated. The GP reserves the right to refuse a request or provide alternative wording if deemed necessary.

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
