You will then visit the practice, bringing with you two forms of identification. One of these items should include your photograph and the other your current address.

We will then issue you a username and password.

If there is any doubt about the patient’s/proxy’s identity, information will not be released until further evidence is provided. You will be informed if this is the case. Under the terms of GDPR, requests will be responded to within 30 days after receiving all necessary information to process the request.

Please note, in order for us to attach proxy access to your existing account you will need to have an online access account yourself. If this application is for a child under 16, the access will cease on their 16th birthday, after which they will need to register for their own account. If this request is for proxy access for a patient over 16 years old, we will need their signature (if possible) and authorisation to provide you with proxy access.

Once you are registered you will be able to use the service to:

• Order your repeat prescriptions
• Make an appointment
• Cancel an appointment
• Change your contact details
• Review your medications and known allergies
