If you have travelled from China or have been in contact with someone confirmed as having Coronavirus in the last two weeks, you may be at risk of Coronavirus. Please do NOT leave the house. Instead, ring 111 if:

- You have been in Wuhan or Hubei Province in the last 14 days or have been in contact with someone confirmed as having Coronavirus, even if you feel well
Important Patient Information
We would like to inform all of our patients that the Sit & Wait clinic will cease shortly after Christmas.
We are currently reviewing and overhauling our appointment system in order to improve access to the surgery. We will provide further information very soon about our future services.

Due to safety reasons we are unable to process any requests for medication over the telephone.
Please note the receptionist is unable to automatically submit requests for prescriptions that are not on repeat. Prescription requests for these items may be refused and you could be asked to make an appointment with the GP, who initially prescribed the medication for you. If your request is for an item that has not been prescribed for you within the last 12 months you may be asked to see a Doctor.
Prescription requests must be requested in writing or by using the repeat prescription slips that are available at reception, alternatively by the practice prescription email: email@example.com.
Requests via email will only be accepted if you have previously provided your email address and this has been recorded onto your clinical record. You must provide the following information:
* Full name
* Date of birth
* Medication required
* Collection method
Patients are requested to kindly give 3 working days (72 hours) between prescription request and collecting the prescription.
Commencing the beginning of June, Dr Fitzgerald is reducing to two working days. She will provide an extended Monday morning service and alternate Tuesdays at Fazeley surgery. She will continue to cover the well-baby clinic Friday morning on a rota basis. Dr Young will reinstate a service at Fazeley surgery most Wednesday mornings. Please enquire at Reception to confirm availability.

Dementia Friendly Communities programme
Alzheimer's Society's Dementia Friendly Communities programme focuses on improving inclusion and quality of life for people with dementia in a number of ways. One of the ways is through the Dementia Friendly Communities recognition process, which encourages communities to sign up and work towards becoming dementia-friendly.
A Dementia Friend learns a little bit more about what it's like to live with dementia and then turns that understanding into action - anyone of any age can be a Dementia Friend. Dementia Friends is about learning more about dementia and the small ways you can help. From telling friends about Dementia Friends to visiting someone you know living with dementia, every action counts.

Following a National Government initiative, from 1st September 2018 extra appointments were offered across the whole of Tamworth (The New Mercian Primary Care Network), including evening and weekend appointments. Shingles Vaccination 2019
The practice will continue to offer Shingles vaccination to those patients who are eligible. The shingles vaccine will be given as a single injection and you will only need to have the vaccination once.

We are unable to provide Hepatitis A for travel vaccination due to a national shortage. We can offer other available vaccinations & any NHS vaccinations recommended for travel.
If you require any travel vaccinations please request a telephone consultation with one of our practice Nurses. The Practice Nurse can discuss your travel destination and discuss with you what vaccinations are recommended.
Please allow enough time to ensure we are able to provide the vaccine/s and allow for the course to be completed prior to your travel arrangements.
If the next available appointment with the Practice Nurse is not soon enough for your travel needs you may need to contact a Travel Service.

Text Messaging Service
You can receive information via Text Message on your mobile phone. We can send a text message to remind you of your appointment date and time. Reminder texts also give you details how to cancel an appointment via text. We can send you reminders regarding invitations relevant to your own health.
It is essential that you give us your up to date mobile telephone number.
If you are unable to attend your appointment please remember to contact the surgery in advance to cancel.
If you are calling re test results, please call 01827 69283 after 2pm.
Please allow up to 7 days for blood/urine results and 2-3 weeks for X-ray / ultrasound results. Some ultrasounds / MRI / CT results can take longer.
